Founded in 1938 by Mrs. Ina Perkins, Perkins Country Manor has cared for Bracken County families for generations. Today we are a skilled-nursing and rehabilitation facility in Augusta, Kentucky.

Our Namesake

Named for Mrs. Ina Perkins

Our story begins in 1938, when Mrs. Ina Perkins — an esteemed member of the Bracken County community — founded the facility that would grow into the home we are today. For decades it served the county as the Bracken Rehabilitation Center.

Mrs. Perkins lived the values we still hold: compassionate care, seeing every resident as a valued and unique individual, treating everyone with respect, and walking alongside each person on their journey to recovery. She set the bar high simply by serving as a role model for everyone who came after her.

In honor of her work and the legacy she left us, the Bracken Rehabilitation Center was renamed Perkins Country Manor — so that her name, and the standard of care she set, lives on in everything we do.
